I'd think canon-Neal must have some sort of health care coverage. If he were in prison, his medical needs would be taken care of, so as long as he's technically in the custody of the state, I'd assume he'd still be covered. (Also, I really can't see Peter putting Neal in dangerous undercover situations knowing there's no way to get him help if he gets hurt.)
